Do Sulfites in Wine Actually Cause Headaches?
Sulfites are one of the most misunderstood ingredients in wine. They are in nearly every bottle you have ever drunk, red or white, expensive or cheap, and the popular theory that they cause the notorious red-wine headache does not hold up well against the actual evidence.
What sulfites actually are and why they're there
Sulfur dioxide (SO2) is wine's oldest and most effective preservative: an antioxidant and antimicrobial that prevents spoilage and oxidation. Crucially, fermentation itself produces sulfites naturally, in small amounts, as a yeast byproduct. There is no such thing as truly sulfite-free wine; 'no sulfites added' wine still typically contains 10 to 40 mg/L from fermentation alone, just below the labeling threshold in most countries.
Why the headache theory doesn't add up
- White wine often has more sulfites than red. White and sweet wines typically need higher SO2 for stability, yet the 'red wine headache' is the famous complaint, not white. If sulfites were the cause, the pattern should run the other way.
- Dried fruit and processed food have far more. Dried apricots can carry 10 times the sulfite concentration of wine. People who react badly to wine rarely react to dried fruit, which is inconsistent with sulfites being the driver.
- True sulfite sensitivity is rare and has a different signature. Genuine sulfite sensitivity, which does exist, mainly affects asthmatics and typically causes respiratory symptoms, not headaches.
What's more likely actually causing it
- Histamines and tyramine. Red wine has substantially more of both than white, from skin contact and malolactic fermentation, and both are established headache and migraine triggers in sensitive people.
- Tannins. Some research links tannin compounds to serotonin release, plausibly connected to migraine in susceptible individuals.
- Alcohol and dehydration. The simplest explanation, often the correct one: alcohol dehydrates and dilates blood vessels, both classic headache triggers, independent of anything else in the glass.
- Congeners. Compounds from fermentation and barrel aging, more concentrated in red and darker spirits, are a known factor in hangover severity generally.

If red wine reliably gives you a headache and white doesn't, sulfites are an unlikely culprit since whites often have more. Histamines, tannins, and simple dehydration are better-supported explanations, and staying hydrated, eating alongside wine, and choosing lower-tannin styles are more useful interventions than chasing 'low sulfite' labels.
